What Are Specialty Drugs?
Specialty drugs that are typically classified based on their high-cost, high complexity and/or high touch characteristics. Specialty drugs are often referred to as biologics which means “drugs derived from living cells” that are injectable or infused; however oral dosage formats have been developed in more recent years. Specialty drugs are generally used for the treatment of complex medical conditions and/or rare conditions and often require specialized storage, handling, monitoring and administration.
Why Are Specialty Drugs So Expensive?
Despite their high cost, specialty drugs are a major advancement in our treatment of complex medical conditions. Despite entering the marketplace over 10 years ago, their extraordinary cost is attributed to research and development and patent protection. The average research and development costs for a manufacture to release a successfully FDA approved drug to the marketplace is in the range of $4 to $12 billion per successfully approved drug. The other important factor is the expiration of drug patents which results in the influx of generic drugs into the market and wreaks havoc on brand drug manufacturer’s revenues. In the United States, the Affordable Care Act (ACA) will incentivize manufacturers to raise their initial pricing for newly marketed drugs as they will be penalized in the future under the Medicaid statutory rebate program if they raise their prices above the consumer price index.
How Do I Get Access to a Specialty Drug?
The majority of specialty drugs cannot be acquired by traditional brick-mortar pharmacies and are generally acquired, dispensed by specialty pharmacies that try to best manage inventory, payer relationships and overall patient service needs.
What Is a Biosimilar?
Biosimilars are often referred to as subsequent entry biologics and have active properties similar to an original patented biologic drug. Biosimilars are NOT generic versions of the originator and pharmacists cannot substitute a biosimilar for the originator. Despite being more affordable, biosimilars have faced challenges with respect to being adopted by patients and their physicians for a number of reasons:
- Physicians are conditioned to prescribe the Originator product and/or reluctant to prescribe a Biosimilar.
- The number of initial approved indications for Biosimilars is typically less than the Originator drug, which ultimately impacts physician prescribing behavior.
- Biosimilars do not provide the magnitude of savings experienced by the introduction of a generic drug.
